• Grandparent Assumption- Must be notarized by the CISD PEIMS Department at 915 Dell Dale St, Houston, TX 77530.

• Parental Assumption-Must be notarized by the CISD PEIMS Department at 915 Dell Dale St, Houston, TX 77530.

Please review the information below to help you determine which affidavit is best for your student's enrollment and the requirements for notary. For more information, please contact your student's campus.

Affidavit Information

• Required to submit all supporting documents 

• Current bills are to be within 30 days of dated application

• Printable affidavits are available online or can be requested from your campus.

• Both parties must be present in order to have affidavit notarized.

Shared Residency Affidavit

The Shared Residency Affidavit is used if the Parent resides with someone in the CISD attendance zone and does not have a lease or bill in their own name. The following attachments are required along with the completed affidavit. 

• Parent’s valid Government Issued Photo I.D. 

• Residency’s valid Government Issued Photo I.D. 

• Residency’s Current Utility Bill 

Grandparent’s After-School Care Affidavit for Non - Resident Student

The Grandparent Affidavit is used only if a parent is requesting a campus that the Grandparent is zoned to in CISD if the grandparent provides before or after school care for the student and  is requiring bus service. The following attachments are required along with the completed affidavit. 

• Parent’s valid Government Issued Photo I.D. 

• Grandparent’s valid Government Issued Photo I.D. 

• Grandparent’s Current Utility Bill 

Parental Assumption Affidavit

The Parental Assumption Affidavit is used if the parent is giving education rights to a person other than the parent, due to the student residing with someone other than a parent. The following attachments are required along with the completed affidavit.

• Biological Parent or Legal (court ordered) Guardian’s valid Government Issued I.D. 

• Person requesting Guardianship’s valid Government issued Photo I.D.

• Person requesting Guardian’s Current Utility Bill

Students currently attending schools in Channelview ISD under a residency affidavit must have their residency verified annually. If you no longer require a residency affidavit, please contact your campus to update your records.
